Supporting our communities together in Wakefield
During COVID-19, partners across Wakefield have worked together to support our most vulnerable residents. One example of this is how general practices have been working with voluntary and community sector to make sure that their patients, who are vulnerable and are shielding, are able to access the holistic support that they require.
In this section of the website there are a number of case studies that share how Primary Care Networks in Wakefield have worked together to support their patients through their enhanced primary care roles that have been created by resources funded through the NHS England Long Term Plan.
The positive impact of the link worker roles for Primary Care Networks is the focus of these case studies.
