Patient Pathway to Employment Service

  • Have you been unable to work for a long period of time?
  • Do you think you will have difficulty finding a job?


If so, you may benefit from a free appointment with a Community Work Coach from the Patient Pathway to Employment Service. 


The Patient Pathway to Employment Service is a free initiative through your GP that can help support you get back into work. Once you sign up, you’ll have an initial chat with a coach who will identify your individual needs and any barriers that may be preventing you from returning to work.  Your coach will then support, guide and encourage you to set your own goals and take positive steps to achieve them.

  • White Rose, South Elmsall
  • The Grove, Hemsworth,
  • King’s Medical, Castleford
  • Church Street, Ossett
  • Chapelthorpe
