Housing Health and Social Care

As a result of the Five Year Forward View and the Next Steps on the Five Year Forward View, the importance of housing in improving health and wellbeing is highlighted.

It is well documented that addressing wider determinants of health can result in a positive impact on the demand for primary and acute services, such as hospitals – whilst improving peoples wellbeing. In March 2017 the Connecting Care Health and Social Care Partnership (CCHSCP) agreed to create a Housing, Health and Social Care Partnership (HHSCP) to sit under the wider architecture of the Health and Wellbeing Board (HWB).

We have representation from Wakefield Clinical Commissioning Group (CCG), Wakefield Council, WDH, West Yorkshire Fire and Rescue Service and the voluntary and community sector. The work of the partnership is also linked as a key enabler in delivering new models of care.
