West Yorkshire and Harrogate Health and Care Partnership
NHS England published guidance in December 2015 asking every health and social care system in communities across England to work together to create their own plan for the delivery of NHS England's Five Year Forward View. These plans are called Health and Care Partnerships (originally called Sustainability and Transformation Partnerships).
The West Yorkshire and Harrogate Health and Care Partnership (WY&H HCP) sees local NHS organisations, councils, charities and partner organisations working closely together to make improvements to the health and social care system.
There are six local councils, eleven Clinical Commissioning Groups (which design, specify and buy care for local communities), hospital trusts, police and fire and rescue services, branches of Healthwatch, charities and many more organisations involved in our partnership. You can view this map to see all partners.
